My Dad was in a mishap in an AD-3W (assigned to VC-12) when they catapulted off Uss Palau in March 1950. I was able to get a copy of the mishap report in 2001, but I was just looking for anyone with any other personal information.
The launch was at night and the report reads the mishap (which resulted in the plane going over the bow, striking the port side forecastle, and going into the water) was cause by imporper placement of the bridle on the shuttle. My father and the pilot survived but the other radar operator was never found. I'm curious to know if anyone else remembers anything about that mishap or the operations that were going on at that time. I think it must have been carrier quals, but I'm not sure of the details. The serial number for the aircraft was 122890.
